Diaverum’s mission is to improve the quality of life for renal patients, constantly striving to provide innovative solutions to enhance our services to our patients. The d.CARE app is our mobile APP designed to empower our patients to take an active part in their health and wellbeing by providing information at their fingertips. Our APP provides dialysis information not only for those patients that undertake HD in our kidney centres but also for Home HD and PD patients.

The d.CARE app contains functionality for My Dialysis treatment information, My Energy and My Bones status. In addition, it is possible to follow the treatment results over time in My Scores section.

The app uses input from the user in the “How I feel” section to graphically present the personal well-being.

A User ID is required for the d.CARE app. User IDs are managed and given out by the local Diaverum clinic.
